class=\"wp-block-paragraph\">In the previous post,<br>I introduced a fundamental principle of surgery:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>\u201cOperate on the area where you want to see improvement.\u201d<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">While explaining this principle,<br>I also pointed out something rather unusual about the procedure called a <strong>facelift<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">And I gave an example of an intuitive wrinkle-removal approach that follows this principle.<br>Let me bring that example back again.&nbsp;<\/p>\n\n\n\n<figure class=\"wp-block-image size-full\"><img fetchpriority=\"high\" decoding=\"async\" width=\"461\" height=\"463\" image<\/strong> shows the <strong>preoperative state<\/strong>,<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">and the <strong>lower right image<\/strong> shows the <strong>postoperative result<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">On the <strong>left side<\/strong>, you can see a preoperative drawing that estimates<br>how much of the <strong>nasolabial fold<\/strong> would be removed during surgery.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Now, this is the question I raised in the previous post.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">If a <strong>direct procedure like this<\/strong><br>is more effective and takes less time,<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Then why <strong>don\u2019t we perform this kind of surgery<\/strong>?<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Why do surgeons instead make an incision near the <strong>ear<\/strong>,<br>and operate on the <strong>nasolabial folds<\/strong> or the <strong>corners of the mouth<\/strong> from there?<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The answer is simple.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Because we must avoid visible scars<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">More precisely,<br>even if we successfully remove the sagging tissue,<br>if a long scar remains next to the mouth or beside the nose<br>that clearly says,<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong><u>\u201cI had surgery,\u201d<\/u><\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Would people really say the result looks natural and beautiful?&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This type of procedure is more selectively considered in patients whose skin tends to form less conspicuous scars.<br>That is why we hide the incision<br>within the <strong>natural shadows and contours around the ear cartilage<\/strong>,<br>and begin the surgery from a point far away near the ear.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">But then what about the fundamental surgical principle I mentioned earlier\u2014<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>\u201cOperate on the area where you want to see improvement\u201d?<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Even if the surgery <strong>starts in front of the ear<\/strong>,<br>it simply needs to <strong>reach the area where improvement is needed<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In other words,<br>we enter through the <strong>front of the ear (the entrance)<\/strong>,<br>then proceed downward <strong>like creating a tunnel<\/strong>,<br>until we arrive at the <strong>nasolabial folds and the corners of the mouth (the destination)<\/strong>.&nbsp;<\/p>\n\n\n\n<figure class=\"wp-block-image size-full\"><img decoding=\"async\" width=\"514\" height=\"514\" to do?<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">As I mentioned in the previous post,<br>if you want to <strong>augment the nose<\/strong>, but instead create an entry point through the <strong>eyelid<\/strong> to get there, the surgery would not only be technically difficult,<br>the result would likely turn out <strong>uneven and unnatural<\/strong> as well.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In the same way, a <strong>facelift<\/strong> also requires traveling a considerable distance<br>to reach the target area.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Because of this,<br>the procedure takes more time, and the surgeon must carefully navigate around important structures that <strong>must not be injured<\/strong> along the way.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">As a result, a high level of <strong>surgical experience and skill<\/strong> is required.<\/p>\n\n\n\n<figure class=\"wp-block-image size-full\"><img 489px\" \/><\/figure>\n\n\n\n<p class=\"wp-block-paragraph\">People in the past, and even more recently,<br>found this to be quite difficult.<br>So instead,they first tried <strong>easier methods<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Do you remember the <strong>tape<\/strong> I mentioned in the previous post?&nbsp;<\/p>\n\n\n\n<figure class=\"wp-block-image 190px\" \/><\/figure>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cIf it\u2019s difficult to reach the <strong>front of the face<\/strong>,<br>why not just <strong>hook onto the sagging area and pull it upward<\/strong>?\u201d<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cIf going all the way from the <strong>front of the ear<\/strong> to the <strong>nasolabial folds<\/strong> is too demanding, why not lift somewhere around the <strong>lateral cheek<\/strong> instead?<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">After all, the face is connected as a whole\u2014<br>wouldn\u2019t the <strong>front of the face<\/strong> be lifted as well?\u201d<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Methods based on this idea were known in the past as the <strong>MACS lift<\/strong>,<br>and more recently, have been widely performed under the name of the <strong>mini facelift<\/strong>.<\/p>\n\n\n\n<figure class=\"wp-block-image size-full\"><img loading=\"lazy\" decoding=\"async\" 284px\" \/><\/figure>\n\n\n\n<p class=\"wp-block-paragraph\">As shown in the illustration,<br>instead of operating on the <strong>front of the face<\/strong>,<br>the idea was to work only on the <strong>lateral cheek area<\/strong> (the gray-shaded region)<br>and lift that part upward,<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">With the expectation that the <strong>front of the face<\/strong> would also be lifted along with it.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">It sounds quite convincing, doesn\u2019t it?<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">And since the more delicate <strong>anterior part of the face<\/strong> was not directly touched,<br>patients experienced <strong>less discomfort<\/strong> as well.<br>In fact, this procedure was widely performed in the past, and it is still performed today.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">However, over time, among patients who underwent surgery using this method,<br>serious problems began to appear gradually.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>What kinds of problems developed?<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">I\u2019ll continue in the next
